Foster home requirements: Provide a save & loving home. Dogs are not to be left outside for extended periods of time. They need to be on a leash at all times while outside in an unfenced area. If you are unable to provide transport, you must agree to make yourself available for the foster dog to be picked up for adoption events and/or vet appointments. All veterinary care will be provided by Semper Fi Legacy Rescue, you are not permitted to take a foster to an unauthorized or authorized vet without approval from a Semper Fi representative. All animals will be spayed/neutered at appropriate age and weight. Before this application can be approved, a home visit must be completed.

By checking the box below, I am attesting to the truthfulness of my answers. Falsification of any of the above information will be grounds for disallowing the fostering of a rescue pet and possible removal of said pet.

Foster Rescue Commitment/Agreement

1. I understand that Semper Fi Legacy Rescue is a non-profit organization devoted to rescuing and placing animals in need. Because of my intention to aid Semper Fi in its charitable pursuits, I agree that Semper Fi and/or representatives are in no way responsible for any loss, damages, injury, or death to any person, animal, or property caused by the Semper Fi affiliated animal(s) placed in my custody.

2. I am aware that Semper Fi affiliated animal(s) may have unforseen illnesses. Further, I realize Semper Fi is only responsible for the medical care of its affiliated animals. I agree that I alone, am responsible for any actions taken by Semper Fi affiliated animal(s) once placed in my custody and under my care. I am aware that to be eligible for reimbursement, all vet visits must be pre-approved by a Semper Fi representative. Emergencies may be considered exceptions.

3. I understand that the veterinarian, Semper Fi, or representatives thereof, take no responsibility as to the character or disposition of the Semper Fi affiliated animal(s) in my custody.

4. Should the Semper Fi affiliated foster animal become lost, stolen, seriously ill, die, and/or bite in an aggressive manner, I will contact the Semper Fi representative immediately. "Contact" shall mean verbal communication by either telephone, text, or in-person between me and the Semper Fi rescue committee chairperson.

5. The foster animal shall wear an ID tag at all times. (DOGS).

6. I understand that should I decide to permanently adopt a Semper Fi affiliated foster animal(s) in my care, that I will pay Semper Fi the adoption fee.

7. I understand that foster animals must attend a minimum of (2) adoptions per month. Exceptions to this requirement would be sickness, animals too young, or animals working out specific behavior issues.

8. I understand that I am fostering the animal for Semper Fi, and although in my care, the animal is under the Semper Fi license and is owned by Semper Fi Legacy Rescue. I also understand that the foster animal must remain in my care and control at all times with the obvious exceptions of pet adoptions, vet visits, and grooming.

9. Once the foster animal leaves the property, I agree to return any Semper Fi property such as crates, food, toys, blankets, puppy pins, etc. and agree to reimburse Semper Fi if said property is damaged for any reason other than from the foster animal(s).

10. I understand that foster animals cannot be released to another party without an approved adoption application and paid adoption fee, or an approved transfer to another foster or rescue individual. "Trial adoptions" are based on circumstances if you feel like it would be beneficial in placing the animal you must have approval from a Semper Fi representative, and all foster homes must be home-visited and approved in advance, as well as specific home placements.

By checking the box below, I certify that I am at least 18 years old and I assume full responsibility for the care and well-being of any animal that I provide foster care on behalf of Semper Fi. Any misrepresentation of the true facts will invalidate my foster home agreement with Semper Fi and will give Semper Fi Legacy Rescue the right to immediately reclaim any or all Semper Fi animals in my care.
