Please carefully read the following:

AGREEMENT

( ) Weekly Fees/Payments must be paid on Saturday by 1 pm, in advance to the up coming week for transportation. After this time, your student runs the risk of not having transportation for the week, being charged a $20 late payment fee to be added to the roster or losing his or her seat.

( ) Monthly or bi-weekly payments must be paid on schedule in advance to service or their account will convert to weekly and the same rules as above shall apply.

( ) Fees are not PRORATED, payment are due regardless if child attends. (Please keep in mind, that the rate that is giving is already discounted because your child is riding as a group)

( ) There is a $35 fee per week for Thanksgiving, Christmas Break, Winter and Spring break and any other full week that the student is out of school during the school calendar year. This payment is due the Friday before the break begins, with a grace period of the upcoming Monday morning. This is a mandatory fee to maintain your seat.

( ) If your child does not need service for a day scheduled for pick up, we require a 24 hour notice. For emergencies, we require a two-hour notice. Failures to do so can result in a $5 add on fee after the first incident, for each time we are not notified.

(  ) Child or Children should be ready to board the transportation upon arrival; Failure to have child ready upon transportation arrival will result into your child missing their ride. When arriving to your home the driver will give two beeps and then wait for two minutes only. After the two minutes is up the driver will beep two times again, before pulling off. WE CAN NOT WAIT.

( )Please have your child ready to walk out the door for the scheduled time that was giving to them. (Due to weather and traffic we cannot always predict the exact time the driver will arrive but having the student ready at their designated time will help the ride go a lot smoother and ensure that they will not be left.)

( )Scheduled time of pick up and drop off can change due to enrollments and withdrawals. Notice of time change will be given through text message or email, one day in advance.

( ) We cannot call while driving. But We do offer a Courtesy text message before leaving the office to let parents know that we are in route (this must be Request by the Parents).

( ) If your child misses his or her ride, if you would like the driver to come back and get them, It will be a $15 fee and it can only be done according to the drivers schedule and if they have time that day. (Please be mindful that we must pick up other students and make sure they arrive at school on time.)

(  ) Children 9-16 years old must have written consent to be dropped off at a home without an adult present. Children 8 years and younger must have an adult present to sign for them when being dropped off at home.

(  )We ask that no outside food enter into the vehicle (unless provided by our company) and no strong perfume, hand sanitizers, hair spray or any other fumes while riding. (We want to protect the children and drivers who have allergies)

(  ) DK- V.I.P. Transportation, LLC. reserve the right to cancel service due to conflicts, lack of area participation, or Child Behavior problem at any time. (We do not give refunds to clients who were cancelled due to behavior problems or for service that have started for that week)

( ) Children 8 years old and older are allowed to use their electronic devices as they please. DK- V.I.P Transportation is not responsible for any damage done to it or if it is lost. We are not responsible for charges made on the device and if it is left on the bus. Students must use earplugs or headphones while riding on the bus.

STUDENT RULES

While in a vehicle, transportation rules must always be followed. Parents, please remind your children of the importance of these rules throughout the year. DK- V.I.P. Transportation, LLC. is transporting the most precious gift of all - your child - and we want to ensure that every ride with us is SAFE! Passengers must always wear seat belts. Passengers will remain seated while the vehicle is moving. (All traffic tickets obtained due to non-seatbelt wearing will be billed to student legal guardian) Passengers should not board or depart from the vehicle until the vehicle has come to a complete stop and the driver has opened the door All passengers will obey the driver during pick up, transport, and delivery. The driver may assign seats, if necessary. No object throwing is allowed in or out of a vehicle. No hanging in or out of the windows while riding in a vehicle. Passengers should not destroy vehicle property or the property of other passengers. (If a student destroys vehicle property, legal guardian will be held responsible for the cost) All passengers must be courteous; inappropriate language/behavior is not acceptable or tolerated. All passengers’ items must stay in their book bags. No food, drinks or candy in the vehicle, unless it is given by the driver.  We are not liable for any missing or damaged items left on a vehicle.

We reserve the right to terminate transportation for disruptive students.

COVID 19-  LIABILITY WAIVER

I further acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that DK-V.I.P. Transportation, LLC cannot guarantee that I will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, drivers, and other students being transported and their families.

I voluntarily seek services provided by DK- V.I.P Transportation, LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while riding in the vehicle.

I attest that:

* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I have not traveled internationally within the last 14 days.

* I have not traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.

* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold DK-V. I. P. Transportation, LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the company, or that may otherwise arise in any way in connection with any services received from DK- V.I.P. Transportation, LLC. I understand that this release discharges DK- V.I.P. Transportation, LLC from any liability or claim that I, my heirs, or any personal representatives may have against the company with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from DK- V.I.P. Transportation, LLC. This liability waiver and release extends to the company together with all owners, partners, and employees.

INSUFFICIENT FUNDS:

There will be a $35 returned fee for all returned payments, plus the amount to cover the check. This payment must be paid in Money order, Cash App, Venmo, Zelle. Parents are responsible for all transaction fees.

If your payment is returned, you will only have the option to pay by cash or money order.

REFUNDS AND CANCELLATION:

Any Customer that enters into agreement with DK V.I.P Transportation, has a one-day grace period for cancellation of the contract and to receive a refund of 75%.

Besides the one-day grace period, we do not give refunds. If you would like to terminate your contract, we ask for a 7-day written notice, emailed dkviptransportation@gmail.com. or text to

PAYMENT AGREEMENT:

Weekly fee MUST be paid on Friday in advance to the upcoming week, by 8pm. If not, your child runs the risk of not being on the roster for the upcoming week and service will be suspended. To resume service, a $20 late fee will be added to your normal weekly rate, and it must be paid before the student receive service. If full payment is not received, along with late fees, services is terminated.

HOLIDAY BREAK PAYMENT
