(my "Canine") and have full right, power and authority to execute this Release. For full and adequate consideration, the receipt and sufficiency of which is acknowledged, I agree as follows:

1. I hereby give my consent and premission to On Holladay Dog Training & Boarding and any affiliate of On Holladay Dog Training & Boarding and any officer, employee, independent contractor, agent or other person acting on behalf or under the authority of On Holladay Dog Training & Boarding, to take photographs electronic images and other recordings of my Canine and/or me, including those which may be personally idententiable. I hereby authorize On Holladay Dog Training & Boarding and each On Holladay Dog Training & Boarding Affiliate (including contactors working on behalf of On Holladay Dog Training & Boarding) to touch up, change, alter or modify in any manner any photograph, electronic image or other recordings, including changes which alter the appearance of my Canine, any person included in the photograph or any object therein.
2. I further hereby authorize On Holladay Dog Training & Boarding and each On Holladay Dog Training & Boarding Affiliate to use the photographs taken of my Canine and/or me for any lawful purpose, including specifically by without limitation for purposes of product packaging, printed material related to products, inserts included in products, and advertising and promotion related to the offering of products and services, including both print and electronic publication, advertising and promotion.
3. I further give you permission to use the photographs and other recordings (whether printed or broadcast, and whether stored in physical form or digitally) of my Canine and/or me in educational materials, publications and presentation by, for and on behalf of On Holladay Dog Training & Boarding or any On Holladay Dog Training & Boarding Affiliate.
4. The foregoing included by is not limited to (i) publication on the internet through websites and social media accounts controlled by On Holladay Dog Training & Boarding, any On Holladay Dog Training & Boarding Affiliate, or any other person, (ii) publication using television and radio broadcasts, CD's, DVD's and other electronic and/or digital media, (iii) publication print, including specifically by without limitation articles and advertisements apprearing in newspapers, magizines, telephone directories, brochures and fliers, and (iv) publication through visual means as part of in-person presentations.
5. I hereby waive and relinquish any right to recieve compensation, financial or in any other form, for the use of these photographs, electronic images and other recordings, other than the consideration (if any) received me prior to the execution hereof. I hereby release and agree to indemnify, defend and hold harmless On Holladay Dog Training & Boarding, and each On Holladay Dog Training & Boarding Affiliate and their respective officers, directors, agents, attorneys, employees and contractors from any liability arising out of or related to the use of the photographs, electronic images and other recordings.
