This agreement relates to services to be rendered in reference to the above named dog. Pet Owner hereby requests On Holladay Dog Training & Boarding to provide certain services with respect to the Pet Owner's Dog as requested by the Pet Owner and agreed to by On Holladay Dog Training & Boarding as described in this Agreement. Pet Owner agrees to deliver and pick up the dog during On Holladay Dog Training & Boarding's normal business hours on the dates indicated in the Pet Owner's reservation, unless prior arrangements are made, and failure to pick up the dog as described in the reservation may result in the accrial of additional boarding and other charges, which shall be paid by Pet Owner. 

On Holladay Dog Training & Boarding agrees to exercise reasonable care to maintain a clean, safe and secure environment for the Pet Owner's dog while the Pet Owner's dog is in On Holladay Dog Training & Boarding's custody. Provided that On Holladay Dog Training & Boarding has exercised reasonable care, On Holladay Dog Training & Boarding shall not be liable for loss or damage to Pet Owner's dog, from any cause, including specifically but without limitation disease, theft, fire, death, escape, or injury. In addition, On Holladay Dog Training & Boarding shall not be liable for harm to persons, other animals or property caused by Pet Owner's dog and Pet Owner shall indemnify On Holladay Dog Training & Boarding from any liability for any damages caused by Pet Owner's dog. In consideration of the acceptance of this registration, and the holding of private sessions and group sessions, and the opportunity to have the dog particiapte, the Pet Owner agrees to hold On Holladay Dog Training & Boarding, and all trainers, independent contractors, employees, assistants, or apprentices, the premises upon which the sessions are to be held, harmless from any claim for the loss or injury, both direct or indirect, to any person or thing by any act of the dog or person while in or upon the premises or grounds or near any entrance thereto. Pet Owner personally assumes all responsibility and liability for any such claim. Pet Owner further agrees to hold the aforementioned parties harmless from any claim of loss of this dog and hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expenses, including legal fees, by reason of the liability imposed by law upon any of the aforementioned parties for damage and expenses. Pet Owner have asked and had answered to my full satisfaction any and all questions regarding my canine’s training, health and safety.

Pet Owner agrees to pay On Holladay Dog Training & Boarding for services rendered based on On Holladay Dog Training & Boarding's standard services rates at the time services are rendered. Pet Owner also agrees to pay, at rates as determined by On Holladay Dog Training & Boarding in its discretion for any other amounts attributable to Pet Owner's dog and the care of Pet Owner's dog, such as special care or handling and other services or supplies provided by On Holladay Dog Training & Boarding. On Holladay Dog Training & Boarding may require payment in advance or at the time services are rendered or may require that payment be made before the Pet Owner's dog is returned to the Pet Owner. Otherwise, Pet Owner will be billed for services pursuant to this Agreement and payment is due upon receipt of the billing statement. Pet Owner will pay all of On Holladay Dog Training & Boarding's expenses in collection, including collection agency fees, attorneys' fees and other costs. On Holladay Dog Training & Boarding may refuse service if billings remain unpaid. Pet Owner hereby grants On Holladay Dog Training & Boarding a lien against Pet Owner's dog and other property in On Holladay Dog Training & Boarding's possesion of Pet Owner's dog until all charges are paid and Pet Owner shall be responsible for boarding charges during the time of possession of the dog by On Holladay Dog Training & Boarding. Private lessons and group sessions are  NON-REFUNDABLE, NON CANCELABLE REGISTRATION. All sessions must be used within 3 months of the purchase date, 24 hour notice is required if needed to rechedule sessions. Failure to do so will result in cancellation of the session with NO REFUNDS OR MAKEUP SESSIONS OF THE MISSED SESSION. Being more than 20 minutes late is considered a no show and cancelation. 

Pet Owner represents to On Holladay Dog Training & Boarding that upon arrival at On Holladay Dog Training & Boarding's facility the Pet Owner's dog is in good health and that all vaccinations are current. On Holladay Dog Training & Boarding may refuse to accept (or may terminate this Agreement and require Pet Owner to immediately pick up) any dog whose health, condition or behavior could jeopardize the health or safety of On Holladay Dog Training & Boarding personnel or other animals. Upon arrival at On Holladay Dog Training & Boarding's facility, On Holladay Dog Training & Boarding will examine the Pet Owner's dog for fleas, ticks and other minor health problems. If health or other problems are found, On Holladay Dog Training & Boarding may in its discretion either (i) bathe and/or treat the Pet Owner's dog at Pet Owner's expense; or (ii) refuse to accept the Pet Owner's dog or return the dog to the Pet Owner until such conditions are corrected. If the Pet Owner's dog becomes ill or if Owner hereby grants On Holladay Dog Training & Boarding the authority, in its sole discretion, to engage the services of a veterinarian of On Holladay Dog Training & Boarding's choice. Charges by On Holladay Dog Training & Boarding or any veterinarian for services shall be paid by Pet Owner. Pet Owner hereby certifies that the following vaccinations are current: Rabies, Bordetella (Kennel Cough) and DHLPP (Distemper, Hepatitis, Leptospirosis, Parainfluenza, Parvovirus). Pet Owner gives consent to On Holladay Dog Training & Boarding staff to administer my canine’s monthly heartworm and flea and tick medications and to communicate with and receive all information from the veterinarian about my canine.

It is understood by On Holladay Dog Training & Boarding and Pet Owner that all provisions of this agreement shall be binding upon both parties thereunto for this visit and for all subsequent visits. This contract, which replaces any prior agreement between parties concerning the subjust matter hereof, contains the entire agreement between parties. Pet Owner has read and understood this agreement and agrees to the Terms & Conditions mentioned above.
