As the legal owner of the above-named dog, I hereby enroll my dog for training and/or boarding with Kali Grimm. This contract is valid from this date forward for any time the dog is in the care of Kali Grimm for training and/or boarding. I understand that my dog will be taken off property for walks and/or training. In the event my dog causes damage to facilities or to Kali Grimm, I understand that I am responsible for said property or injury.

Kali Grimm will endeavor to create as safe an environment as possible for the boarding/training of my dog and will offer only sound, safe, and responsible care, training and training instructions. However, I recognize that Kali Grimm is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

VACCINATION INFORMATION

For the safety of all dogs in the care of Kali Grimm, every dog is required to have the following vaccinations or titers:

Current rabies (1 year or 3 year)
Bordetella
Distemper/hepatitis/parvovirus (often given in combination as DHPP, DA2PP, or similar)

Proof of vaccination or titers as well as current veterinarian information must be provided before training and/or boarding. These must be received at least 24 hours prior to your appointment. You are responsible for providing up-to-date records prior to your appointment and understand that if records are out of date, we will not perform any services to your pet & are subject to rescheduling fees outlined below.

EMERGENCY VETERINARY CARE ACKNOWLEDGEMENT

Even though I use extreme caution and care in all situations, possible problems could occur. In the event that an accident does occur, this agreement gives Kali Grimm permission to obtain immediate veterinary treatment for your dog should it be deemed necessary. You will be contacted as soon as possible regarding the situation.

If agreed to below, I acknowledge that Kali Grimm may administer first aid, CPR, and/or take my dog to recieve emergency care if needed. If taken to a vet, I will be responsible for all or part of the cost of the evaluation or treatment.

If your dog has a medical emergency, and we are unable to reach you, please select one of the following:

I authorize all possible lifesaving & comfort treatments to be given to my dog. I expressly give permission for emergency veterinarians and care staff to determine the best care plan for my dog.

I do not allow Kali Grimm, their affiliates, emergency veterinarians, or any other party to provide medical care to my dog(s).

NO-SHOWS & CANCELLATIONS

*PAYMENT IS DUE AT DROP OFF* You may cancel or reschedule your appointment reservation without penalty 1 business day prior (at least 24 hours' notice). No-shows or cancellations within 24 hours of the appointment are subject to a fee of 50% of the cost of the planned services (ex: a $100 fee for a $200 training session). If you need to cancel or reschedule for any reason, please call or text immediately with as much notice as possible. I understand there may be emergency situations with extenuating circumstances, and I will do my utmost to work with you on a case-by-case basis, I ask that you communicate with me as best you can and be considerate of my scheduled time. I understand that if I do not provide at least 24 hours’ notice for appointment cancellation/reschedule, I am subject to a charge of 50% of the cost of planned services to the card on file.

I understand that all of these fees are PER dog & will apply in multiples if I have appointments for more than one dog.

BITE DISCLOSURE

You must inform Kali Grimm if your dog(s) has ever bitten a human, dog, or any other animal. Muzzles may be used in these instances to provide safety to Kali, other humans, and other animals. Kali Grimm reserves the right to refuse/ stop services for a such dog(s) at any time before or during the service. Owners are legally obligated to inform me of an aggressive/dangerous dog. If you knowingly fail to do so, you may be liable for any injury and/or damages that your dog causes while in my care.

Bite Disclosure Acknowledgement

My dog has bitten another human, dog, or other animal.

My dog has a documented bite history and has been deemed dangerous.

PHOTO AND VIDEO RELEASE

I authorize Kali Grimm to take photos and video of my dog while in their care. I grant Kali Grimm permission to use these photos and video on Facebook, Twitter, Instagram, and other social media platforms for advertising and marketing purposes. I understand that my name or contact information will not be disclosed at any time.

HOLD HARMLESS AGREEMENT

By submitting this contract, I (or your agent) certify that I have been told and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself and others. Further, I agree to hold Kali Grimm harmless from any damage, loss, or claim arising from any condition of the above-named dog, either known or unknown to Kali Grimm, except in the event of gross negligence. It is also further understood and agreed the terms of the agreement can change at any time, in which case you will be notified and will overwrite any and all prior signed contracts or releases. It is further understood that this clause applies to all dogs under Kali Grimm's care. By submitting this contract, I represent that I have read, understand and accept the terms and conditions stated in this agreement, and acknowledge that this agreement shall be effective and binding upon the parties.
