➢ Learner’s Health Form 14 (completed by child’s physician) 

                  ❖ Learners turning 5 years old and learners entering 7th grade will need updated physical and vaccination records 

➢ TB Reading results (if coming from another country or state) 

➢ Latest report card and/or release card from the last school attended.
