We intend for your experience to be professional, relaxing, enjoyable and guarantee service until satisfaction. Teamwork is our #1 asset which ensures our guests always feel important! We welcome any feedback upon completion of your service. Please read and agree to the following terms as it pertains to your hair extension install. (A written copy will be required signed in person).

I acknowledge that the service is final after the installation and there are no refunds for services rendered. If there is any issue with the comfort or functionality of my extensions, I agree to contact the salon for any adjustments within 48 hours. Any changes to style that I would like to achieve after the installation will additionally be charged accordingly.

I acknowledge that hair extensions are very sensitive and different from intact human hair. I have been informed of the daily maintenance procedure and will follow the daily maintenance procedure to keep my extensions in the best condition as possible.

I acknowledge that I have inspected and approved the hair extensions that are to be installed in my hair.

In the event that I decide to no longer keep the hair extensions, I am fully responsible for the total payment of services rendered. I also understand the explanation of the entire procedure, and am aware that with proper care on my part, they should remain in my hair for at least ________ days/weeks. I understand that if an allergic reaction occurs, I will not hold Chic Haus Salon + Extensions or the stylist at fault.

The charge for the removal of the extensions is not included in the original fee.
