PERSONAL INFORMATION

DRUG/ALCOHOL TEST

I understand and agree that the management of Zummo Meat Co., Inc., may request that I submit to a drug/alcohol test as the abuse/presence of drugs/alcohol may have an adverse effect, among others, on safe working conditions, increase the risk of injuries to myself and/or co-workers, increase absenteeism, increase the risk of theft, decrease associate morale, decrease productivity, and may also result in a decline in the quality of products and services. A drug/alcohol test will be conducted by Zummo Meat Co., Inc., on the Company premise and/or a recognized testing company that normally conducts such testing as a usual business activity. I understand that the results of such a drug/alcohol test will not be revealed to anyone except the management of Zummo Meat Co., Inc and all records concerning test results will be kept in the medical file that is maintained separately from the personnel file. I understand that Zummo Meat Co., Inc., may request a drug test for drugs/alcohol before offering me employment with Zummo Meat Co., Inc. I understand that I will no longer be a candidate for hire if I fail to receive an acceptable result from the pre-employment drug test conducted for me or for failure to submit to the requested pre-employment test. I understand that while I am under no obligation to disclose my prescribed medication, it’s in the best interest that you advise the testing lab of all prescription drugs taken in the past month before the test and be prepared to show proof of such prescriptions to testing lab personnel because an unacceptable test may result in ineligibility as a candidate for hire or may result in my termination if employed. I understand that after I am employed by Zummo Meat Co., Inc., I may be discharged for failing to receive an acceptable result from any drug/alcohol test conducted for me or for failure to submit to a requested test. I understand that the Company request such information as a part of its continuing effort to maintain the highest quality safe work environment. Testing Methods and Procedure Upon receipt of an offer of employment, candidates must complete required drug testing within 24 hours. All testing will be conducted by a licensed independent laboratory, which will follow testing standards by state law. Testing will be conducted on a urine sample provided by the employee to the testing laboratory under procedures established by the laboratory to ensure the privacy of the employee while protecting against tampering/alteration of the test results. Refusal to Undergo Testing Candidates who refuse to submit to a drug test or who fail to show up for a drug test within 24 hours of an offer of employment will no longer be considered for employment, and any offer of employment will be rescinded. Right to Explain Test Results All candidates have the right to meet with the testing laboratory medical review officer (MRO) with Eastex Urgent Care or Nova Medical Centers to explain their test results. These discussions will be considered confidential except that information disclosed in such tests will be communicated to Zummo Meat Co., or with the lab medical review officer (MRO) who needs to know such information to make proper decisions regarding the test results or employment of the individual.
