I/we give permission for my child to participate in birthday party activities being held at Happy Girls Party Boutique,LLC . I understand that my child will be supervised Happy Girls staff during the duration of the birthday party, and at no given time will any child be permitted exit or be released without an adult.

Food Allergy Permission

I/we acknowledge that my child will be participating in various activities at Happy Girls Party Boutique. Additionally, some of the activities involve food products as part of the experience. Happy Girls staff takes precaution to ensure that all particapants are monitored and all food and beverages are properly handled. Happy Girls Party Boutique, LLC, will not be responsible for any allergic reactions.

I /we understand If my child has any known food allergies that may prevent her/him from participating in these activities, we will provide an alternative option per your request.

Service Consent

As part of the Happy Girls party experience, age appropriate makeup, nail polish and all natural spa treatment products will be applied.

I/we understand Happy Girls Parties are for entertainment purposes only, they do not and will not claim to be licensed nail technician or estheticians. At no given time will party host cut cuticles or use harsh chemicals on any child. All Happy Girls staff will exercise special care in being sanitary and using mild kid-friendly products that are especially sensitive to adolescent skin.

Photo Release for Minors

I/we give Happy Girls Party Boutique, LLC permission to use my child’s likeness, image, voice, and/ or appearances as such may be embodied in any pictures, photos, video recordings, digital images, and the like, taken or made by Happy Girls Party Boutique, LLC.

I/we agree that Happy Girls Party Boutique, LLC has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Happy Girls Party Boutique, LLC business. These uses include, but are not limited to, reprints, publications, advertisements, social media and any promotional materials in any medium now known or later developed, including the internet.

I/we acknowledge that I/we will not receive any compensation for the use of such pictures, etc., and hereby release Happy Girls Party Boutique, LLC , from any and all claims which are in any way connected with such use.
