Welcome Packet - Child Care Center/School

We are so excited to become a part of your child's growth and development! We will conveniently provide services to your child at their child care center, school, or after school care locations. Please read through this packet and provide attestation through initials and signatures where indicated.

Our ST, OT, and PT departments work as teams to ensure that every child receives their prescribed minutes of therapy every week. This means that your child might be seen by multiple therapists on any given week. You might also receive weekly communication from multiple therapists.

You will receive an invite to Fusion Parent Portal where you can log in and see appointments and review Evaluation Reports.

PlayRx uses BloomText to send secure chat links right to a text thread or email. No need to download an app, just click the link sent to be connected directly to your child's therapist.

PlayRx - Attendance and Communication Policy - Childcare Center/School

It is our mission to provide services that are convenient for caregivers, by providing clinical services to children who attend daycares. In return, we are asking that the following instances be communicated to your child's therapist or directly to our office through Phone call, Text, or Email. Our therapists work their schedules around the presence of every child on their caseloads, and to ensure that your child will receive all their prescribed minutes of therapy each week and that we are providing the appropriate care given your child's specific medical situations, we are needing parents and caregivers to agree to enhance communication regarding the following: *please initial each of the following:

I will alert PlayRx when my child will be absent from their childcare center for: Appointments, Sickness, Vacations, Holidays

I will alert PlayRx anytime there is a change in my child's drop off and pick up times.

I will alert PlayRx prior to my child leaving their current center to attend another center, go to kindergarten, and/or to stay home.

I will alert PlayRx of any change in the Medical Status of my child: Change in PCP, addition of Medical Diagnoses, change of medications, addition of specialty providers, medical reports: swallow study, hearing/psych/educational/allergy testing, etc

I understand during closures of daycares, holidays, or during any other instances when my child cannot be seen at their current center, PlayRx will attempt to schedule my child for outpatient services.

I will alert PlayRx of any change in my contact information.
