PHOTO RELEASE

I hereby authorize Christian Outreach in Action the right to take edit, alter, copy, exhibit, publish, distribute, and make use of any and all pictures of videos taken of me by Christian Outreach in Action to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters annual reports, press kits, and submissions to journalists, websites, social networking sites, and other print and digital communications without payment or any other consideration. This authorization shall continue indefinitely unless I otherwise revoke said authorization in writing.

I understand and agree that these materials shall become the property of Christian Outreach in Action and will not be returned.

RELEASE AND WAIVER OF LIABILITY

I hereby for myself release, forever discharge, waive and relinquish any and all actions, claims or arising out of the course and scope of my volunteer service against Christian Outreach in Action and/or employees. It is my intention to exempt and relieve COA from any and all liability to me, my heirs. administrators, executors and assigns for bodily injury , property damage, and wrongful death caused by negligence.

I hereby hold harmless, and release Christian Outreach in Action from liability, petitions, and accuses of action which I, my heirs, representatives, executors, administrators, or any other persons may make while acting on my behalf or on the behalf of my estate.

I warrant that I am of the age of consent (18 years or older) and that I am competent to contract in my own name. I have read this release before signing below and fully understand the contents, meaning and impact of this release.

VOLUNTEER AGREEMENT. GUIDELINES AND POLICIES.

All volunteers must sign-in. If scheduled to volunteer and are unable to make it, please let us know as soon as possible (this allows those on the waiting list to sign-up, and assists the supervisor in planning). While volunteering, you must wear a vest at all times. All on-site volunteers must use the restrooms located outside in the back (locate staff for key). COA is not responsible for personal items brought into the building. Volunteers may not take food items unless approved by supervisor. Being under the influence of drugs and alcohol will result in immediate dismissal. Attire should be conservative and suitable, causual dress suggested. Includes proper personal hygiene. Treat all clients, staff, and other volunteers with respect and dignity. Volunteers shall not share unauthorized COA trade secrets or confidential information. Any violation of this policy will result in immediate removal from volunteer duties.
