A. DATA PRIVACY

  • By submitting this form, I consent to the viewing and processing of my personal and family information by the school administrators of Studentdesk Integrated Montessori School, who manage the Application Form. I acknowledge and agree to provide Studentdesk Integrated Montessori School with my personal and family information as defined by the Data Privacy Act of 2012 (R.A. 10173). This information will be processed in relation to my child's admission, enrollment, academic pursuits, and other associated activities at SD IMS.

B. IMPORTANT GUIDELINES

  • Please have the following requirements due upon application ready before filling out the online application form: (a) PSA Birth Certificate (applicable to all student applicants) (b) Latest Progress Report with LRN -applicable for returnee and transferee applicants only.
  • Kindly refrain from refreshing the page while filling up the online application form. This will return you to the first page of the Admissions, and any changes made to the form will be lost.
  • Any changes in your contact information, after the submission of your application, should be relayed to the SD IMS Admissions Office. You may e-mail admissions@studentdeskims.org. TYPE N/A IF NOT APPLICABLE.
  • Accepted file types are compressed images only (jpg, png).
  • Each requirement is limited to one (1) file upload. If there is a need to upload multiple files for a specific requirement, kindly merge them into one file.
  • Scanning physical documents is ideal. In absence of a scanner, you may instead use a photography device.
  • All fields marked with a red asterisk (*) are required fields that should be given values, otherwise, the system will prompt an error message.

C. SUBMISSION ACKNOWLEDGEMENT

  • Please ensure that the email address you provide here is active and accessible, as this is where the system will send the confirmation of your submission along with any instructions.
  • Our Admissions Staff will review them thoroughly and reach out to you if there are any concerns regarding the submitted documents.

D. MODIFIED BUSINESS HOURS

  • Kindly take note that submission/s received beyond the official working hours (Monday-Friday, 9:00AM-4:00PM) and holidays may take us longer to respond.

  • I hereby certify that all information requested and submitted via the Studentdesk Integrated Montessori School (SD IMS) online application form is complete and accurate.
  • I authorize SD IMS to verify all information supplied in the form.
  • I give SD IMS full consent to use, share, disclose and transfer data in the form for purposes related to my application.
  • I understand that any misrepresentation of information will be considered sufficient reason for refusal of admission and/or future exclusion.
  • Should my child be admitted, I will submit to SD IMS the enrollment requirements of my child during enrollment or as soon as available. 
  • I understand further that failure to fulfill the aforementioned requirement before the beginning of academic year shall render the enrollment invalid.
  • It is the responsibility of the student applicant and the parents to ensure the authenticity of the documents submitted.
  • SD IMS reserves the right to: (a) Cancel the admission of the student if after enrollment, the documents submitted were proven to be fake, tampered, or manipulated. (b) Require resubmission of documents due to poor image quality and readability. (c) Request for supporting documents of the ones submitted previously.
  • As per Article V, Section C, Page 8 of the enclosure to the Department of Education Memorandum No. 3, Series of 2018; otherwise known as the Basic Education Enrollment Policy; all student-applicants will be considered on temporarily enrollment until such time that all original documents are submitted.
  • Parent/s or the duly authorized guardian of the student-applicant is required to submit an Affidavit of Undertaking for the student-applicant to be temporarily enrolled.
  • Failure of the student to submit the original documents may have the following consequences: (a) Non-enrollment in the DepEd’sLearner Information System (LIS) for Basic Education Program; (b) Future problems in the processing of school credentials.
