1. The practice of Aerial Yoga and relying on the Aerial Hammock has certain hazards and risks and it requires physical exertion that may cause physical injury or cause aggravation of pre-existing conditions.

2. I have been advised to consult with a medical physician prior to joining an Aerial class, workshop, or any Aerial session.

3. During any instruction from the leader, I understand that I have the option to opt-out by letting the instructor know my wish not to have physical adjustments/spotting.

4. I hereby release, waive, discharge, and hold harmless the institution, Your Personal Hype Woman or staff, affiliates, and partners from any and all liabilities arising from any untoward incident in my participation in any Aerial class, workshop, and relevant sessions using the apparatus which may result to injury, loss, damage, or death.

5. Classes may be recorded or photographed. Your Personal Hype Woman has complete ownership of such material, and content and can use said material or content for any marketing or promotion purposes on any social media platforms.

By signing this form, I hereby represent and warrant that I am physically fit and capable to participate in Aerial classes, Aerial workshops, or Aerial activities. I agree and legally bind myself, with the full understanding of the contents and meaning of the provisions above. I declare that I am over 18 years of age and fully capable of giving my consent.
