After having been sworn to in accordance with law, do hereby depose and state that:

1. I was a parent of STUDENTDESK INTEGRATED MONTESSORI SCHOOL INC. (SD IMS), and was issued an ID.
2. Unfortunately, said ID inadvertently got lost/missing.
3. In spite of all efforts to recover the said ID, the smae was nowhere to be found and should therefore be considered lost.
4. I am executing this affidavit to attest to the truth of the foregoing and for wahtever legal purpose it may serve.
