If needed, please consider adding an additional 1/2" - 1" to your measurements to allow for a comfortable fit. ***IMPORTANT NOTICE*** 

For clients whose prom is just a month away, it is still possible to create a custom dress. However, please note that during prom season, our stores experience a high demand for materials, resulting in daily depletion of stock. In order to provide you with the dress of your choice, it is essential that you have a clear vision of what you desire and are open to compromising on the available materials in-store. 

We always strive to accommodate our clients' preferences and will do our best to source the required material if it is in stock. In the event that the desired material is unavailable, we will work closely with you to explore alternative options. We understand that some clients may have specific fabric requirements and if you are unable to compromise, we completely respect your decision. Regrettably, we will not be able to proceed with creating your dress under such circumstances. 

To ensure smooth communication and quick response times, we kindly request that you enable notifications on our Instagram page @shopdamik. This will play a crucial role in facilitating effective communication, particularly for rush orders.

We hope you are eagerly anticipating the upcoming prom season! We would like to provide you with a breakdown of the payment plan for your convenience. 

Let's consider a dress priced at $1,200.00. To secure your spot, an initial payment of $100.00, resulting in a remaining balance of $1,100.00. (ONLY IF THE $100 FEE IS STILL AVAILABLE)

To commence the dress creation process, a first payment of $350.00 is required, leaving a balance of $750.00. 

For the second payment, an amount of $400.00 is requested, resulting in a remaining balance of $350.00. 

Lastly, a third payment of $350.00 is required, after which the remaining balance will be cleared, resulting in $0.

We understand that everyone's financial situation may vary, and we are here to accommodate your needs. If you have any specific payment preferences or require assistance, please do not hesitate to let us know. To ensure the smooth and timely completion of your order, we kindly request your full attention to the following payment terms. 

Please be aware that a $200.00 rush fee will be applied to your invoice if the full payment is not received by the designated deadline, which is 15 days prior to the event day. It is imperative that the invoice is settled in its entirety before the specified deadline, as indicated on the invoice itself.

We would like to emphasize that any delay in payment, even by a single day, will result in the imposition of the $200.00 rush fee. This fee is necessary to accommodate the necessary adjustments in scheduling and to ensure that all orders are processed efficiently on a daily and hourly basis.

Directions on how to pay on website.

1. Go to "www.shopdamik.com"

2. Visit the "Shop Dresses" or "Shop Dresses"

3. Go to "Payments for prom invoice" 

4. Add up the amount for which you are paying. 

5. In the notes, put the name from the invoice of which you are paying for so that the money is going to the correct invoice. If you are paying for Jane and I have multiple Jane invoices it will not help without a last name.

If invoice deposit is not paid within 24 hours, this form and invoice will be deleted and you will have to resumbit a new form.

We are only taking clients who are serious about booking and starting the process on the custom dress orders.

We are currently helping a maximum 180 girls this season and we strive to give quick customer service to those in need. 

If you are not commited to booking, please do not submit this form. 

Any items adorned with rhinestones or feathers, you acknowledge and agree to how use permanent adhesive. This is necessary to avoid any potential complications on the day of the event, such as the  the inadvertent detachment of heavy panels, which could result in an inappropriate exposure.
