Cornerstone Thai Boxing & Women's Training Center Waiver and Release of Liability Terms & Conditions.

I, the undersigned, agree to participate in the classes, practice self-defense techniques, martial arts, open mat and fitness activities offered by Cornerstone Thai Boxing, LLC, a New York limited liability company, d/b/a Cornerstone Thai Boxing, (hereinafter “CTB”) and Women’s Training Center Inc, (hereinafter “WTC”) located at 42-18 162nd Street, Flushing NY 11358 to release, waive, discharge, and covenant not to sue, and agree to hold CTB and WTC, its owners, managers, directors, trustees, officers, servants, agents, volunteers, and employees against any and all liabilities, demands, damages (economic and non-economic), claims, or injuries, including death, and any other rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from, arising out of, or in conjunction with the activities. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the CTB and WTC, its owners, managers, directors, trustees, officers, servants, agents, volunteers, and employees either on- or off-site.

Assumption of the Risk Relating to Coronavirus/COVID-19

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the CTB / WTC classes and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at CTB / WTC classes may result from the actions, omissions, or negligence of myself and others, including, but not limited to, CTB / WTC owners, managers, directors, employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the CTB / WTC or participation in CTB / WTC programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the CTB / WTC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the CTB / WTC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any CTB / WTC program.

I grant to CTB / WTC , its representatives and employees, the right to take photographs, record video, and record interviews and quotes. I authorize CTB / WTC , its assigns or transferees to copyright, use and publish the same in print and/or electronically. I agree that CTB / WTC may use such photographs, videos, and quotes with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, social media and web content. I hereby release CTB / WTC  and its representatives from any and all claims whatsoever in connection with the use, reproduction, and publication of the images, videos, interviews, and quotes thereof.

I have read and understood the foregoing assumption of risk, and release of liability and I understand that by signing it obligates me to indemnify the parties named for any liability for injury or death of any person and damage to property caused by negligent or intentional act or omission. I understand that by signing this form I am waiving valuable legal rights.
