I understand that any misrepresentation of fact may result in a refusal of adoption privileges. I authorize Active Paws Rescue to contact all persons listed on the application. I fully understand that a home visit may be requested by the Foster of the Animal or a Board Member. If my request for adoption is approved and later Active Paws Rescue discovered the above information is not true or correct, I understand Active Paws Rescue reserves the right to require return of the adopted pet.
