I hereby enroll my child/self in Kinetic Movement Center classes and agree to pay tuition and fees specified for those classes. I am the legal guardian/representative and forever release, discharge, acquit and hold harmless, Kinetic Movement Center including Tindal Jordan and all their employees, teachers, agents from any/all damages or injuries of any kind, nature or description, resulting from classes, performances, outside play area, or other events with Kinetic Movement Center. I acknowledge the fact that certain types of injuries are common and inherent to dance related activities.  This release includes, but is not limited to, that type of injury.  I hereby give permission for images captured of my child/self, during regular or special dance activities through photo, video, or digital camera, to be used solely for the purpose of Kinetic Movement Center promotional material and publications, and waive rights of compensation or ownership thereto.  This release shall be binding upon and inure to the benefit of the parties, their successors, assigns, and personal representatives.
