Football sessions are held once a week in six week blocks at Oaks Park High School from 6.00-7.00pm. For those wishing to take part in Tuesday football sessions we ask for a payment via Standing Order of £30 per block to help us cover our costs. If you cannot contribute this amount then please speak to us directly.

Does your child suffer from any medical conditions or allergies? If so, please provide further information. Medication must be brought along to training if required. Failure to do so may result in child being unable to participate in activities.

Does your child have any additional needs of which Onside staff should be aware?

Occasionally Onside Soccer will take photographs/video of training sessions which may be used for promotional or training purposes. Names of children will never be published. Do you consent to material being used in this way?

In the event of accident or emergency do you consent to Onside Soccer staff providing basic first aid to your child? (e.g. ice-pack, sticking plaster, etc.)

In an emergency, if I cannot be contacted despite all reasonable attempts to do so, I give permission for my child to undergo emergency medical/dental treatment including the use of anaesthetics as considered necessary by the medical authorities.

As a charity Onside Soccer relies heavily on donations to support our work. If you donate to any of our projects and you are a U.K. tax payer we can claim Gift Aid on your donations, which makes your support go even further. To allow us to claim Gift Aid, please read and complete the section below.

I am happy for Onside Soccer to claim Gift Aid on any donations I make to the charity. I confirm I have paid or will pay an amount of Income Tax and/or Capital Gains Tax for each tax year (6 April to 5 April) that is at least equal to the amount of tax that all the charities or Community Amateur Sports Clubs (CASCs) that I donate to will reclaim on my gifts for that tax year. I understand that other taxes such as VAT and Council Tax do not qualify. I understand the charity will reclaim 25p of tax on every £1 that I give on or after 6 April 2008.
