Medical Release and Authorization || As Parent and/or Guardian of my child or ward, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named child. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to Ashfield Farm and its affiliates including Directors, Leaders, and Farm Staff to provide the needed emergency treatment prior to the child’s admission to the medical facility.Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Ashfield Farm will be providing care for your child from the set hours on the specific days that you are registered. A fee of $5.00 per ¼ hour will be charged for any late pick ups outside of the set programming hours and Before/After Care. Daily programming will include: group activities, horseback riding, interactions with animals, free play, meal times, and creative crafts. Ashfield Farm will provide a safe, nurturing environment with high quality and developmentally appropriate programming based on farm values, animals, nature, and outdoor adventures.

• Tickets have no cash value.
• Registrations are specific to each week. We are able to transfer your registration to another week IF there is availability in that week.
• Farm Camp weeks are only valid for the 2025 summer season, week(s) cannot be transferred to the following year.
• All sales are final and promotions, discounts and coupon codes cannot be added retroactively.
• We are open snow, rain or shine. Tickets will not be cancelled, transferred, or refunded due to weather.
• Ashfield Farm reserves the right to refuse entry to anyone they deem unsafe to its guests or staff.
• Image release || When you enter Ashfield Farm or an Ashfield Farm event, you enter an area where photography and video recording may occur. By entering the event premises, you consent to photography, video recording and its/their release, publication, exhibition, or reproduction for news, webcasts, promotional purposes, advertising, on websites, social media, or any other purpose by Ashfield Farm.  By entering the event premises, you waive all rights or claims for payment in connection with any use or publication of these materials, regardless of the purpose or sponsoring of such use or other publication irrespective of whether a fee for admission, registration or sponsorship is charged.

Ashfield Farm is NOT responsible for the following: Applying or giving medications, ointments, prescriptions or providing any other type of assistance in medical care for your child (except for in a medical emergency where first aid and or CPR procedures must be applied). 

Parent’s Responsibilities: Payment is expected whether the child is in attendance or not. Payment will not vary due to weather related circumstances, child’s illness, doctor appointments, or parent holidays. 

What to Provide: You are responsible for bringing/providing any necessities your child may need including appropriate attire, a light nutritious snack and lunch (if in full day programming). Please note, Ashfield Farm is a nut-free facility. 

Sick campers are to remain at home if they have a fever, a known contagious illness, head lice, if the child is vomiting, or has diarrhea. Please contact us if you will be keeping your child home on any particular day. Ashfield Farm reserves the right to send children home if they are deemed too ill to participate.

Conditions of Registration + Liability Form ||

1/ The parents/guardians submitting this application are those having legal custody over the child. Conditions of custody, if applicable, will be fully communicated in writing to Ashfield Farm, including a photocopy of the section of any court order referring to visitation rights.

2/ The Camp Director reserves the right to dismiss a camper who, in the Director’s opinion, is a hazard to the safety and rights of others, or who appears to have rejected the reasonable controls of camp and/or whose behaviour is placing unacceptable constraints upon the staff and their responsibilities toward other campers. This may range from missing an activity to complete dismissal depending on the situation. In the instance that a camper is dismissed from camp for any reason, the parents/guardians are responsible for pick-up of the dismissed camper with no entitlement to a refund.

3/ I, the parent/guardian of the herein named participant, release Ashfield Farm, its trustees, directors, corporation members, staff and agents from any loss, personal injury, accident, misfortune or damage to the herein named or his/her property, with the understanding that reasonable precautions shall be taken to ensure the health and safety of the herein-named camper. Each camper must be covered by Provincial Health Insurance or equivalent medical insurance prior to arrival at camp and must extend throughout their entire time at camp.

4/The parent/guardian agrees to permit reasonable use of photos, videos or other pictures of the applicant camper in promoting Ashfield Farm and/or camp activities and programs, and/or Ashfield Farm in general in printed and/or electronic media.

5/ I give my permission for Ashfield Farm to communicate camp information or registration opportunities to me electronically.

6/ The parents/guardians hereby agree to reimburse Ashfield Farm for any willful damages caused by the applicant camper.

7/ The parents/guardians acknowledge the inherent risks of swimming, horseback riding and visiting a working farm with large animals and machinery. The parents/guardians recognize the inherent risks of injury involved in any type of pool, animal or farm activity. Animals do have minds of their own. No animal is a completely safe animal.  If an animal is frightened or provoked it may divert from its training and act according to its natural survival instincts; which may include but not limited to: stopping short, changing direction or speed at its own will, shifting its weight, rearing, biting, bucking, kicking, or running from danger. 

8/ In partaking in programming at Ashfield Farm, I assume any such risk of injury and further, I voluntarily release Ashfield Farm, its instructors, managers, volunteers and agents from any responsibility on account of any injury, I or my child or ward, may sustain while receiving instruction, while swimming or while riding in connection therewith, and I agree to indemnify and hold harmless Ashfield Farm, its instructors, managers, volunteers and agents on account of any such claim. 

9/The parent/guardian agrees to be responsible for the payment of all fees due to the Camp. This registration is not complete until all fees are paid.

10/ No admittance will be given unless this contract is completed in full.
