I. What is health coaching?
Coaching is a dynamic partnership between the client and the coach whereby the coach assists the client in setting goals to develop and potentially achieve career, personal, or professional goals. This process is creative and explorative as the client seeks to ask the bigger questions that might lead to success in small and large goals.
II. Coach and Client Responsibilities
A. I, the Coach, will:
- Ask powerful questions
- Listen without judgment
- Assist you in setting goals, planning and strategizing
- Hold you accountable for what you want to achieve
- Have belief in you as a capable and creative person
- Be your guide by the side
B. I, the Coach, will not:
- I will not be your therapist as I am NOT a licensed therapist.
- I will not do your work for you
- I will not set your goals for you
- I will not judge you
- I will not give you advice
- I will not take responsibility for you or your actions
C. You, the Client, understands that you are responsible for any choices or outcomes made during our time in the defined length of the coaching relationship. The Coach is not liable for any outcomes derived as a result of our coaching processes or sessions. You will do your own work in between our sessions as these are your goals and therefore you are responsible for all work you choose to do.
D. You, the Client, understands that this process can be lengthy, as desired outcomes do not happen overnight. As such, during this process, I, the Coach, might ask you questions about your family, relationships, health history, recreation, and fitness, with the understanding that any information derived from our partnership are asked to enhance the creative process in order to assist you, the client, in achieving successful results.
E. You, the Client, acknowledges that I cannot and will not diagnose and/or treat mental illnesses or conditions therein. If you are seeing a mental health professional, please inform your therapist of the nature of our partnership to ensure that we are working within the safest and healthiest parameters for your emotional and mental well-being.
III. Confidentiality
This coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality set forth in the ICF Code of Ethics. However, please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent. The Coach will not disclose the Client’s name as a reference without the Client’s consent.
Confidential Information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Client; (b) is generally known to the public or in the Client’s industry; (c) is obtained by the Coach from a third party, without breach of any obligation to the Client; (d) is independently developed by the Coach without use of or reference to the Client’s confidential information; or (e) the Coach is required by statute, lawfully issued subpoena, or by court order to disclose; (f) is disclosed to the Coach and as a result of such disclosure the Coach reasonably believes there to be an imminent or likely risk of danger or harm to the Client or others; and (g) involves illegal activity. The Client also acknowledges his or her continuing obligation to raise any confidentiality questions or concerns with the Coach in a timely manner.
IV. Description of Services
A. The Client agrees to enlist the help of the Coach for 3 or 6 months of coaching. The Coach and Client will meet either every week or every two weeks, as determined by the Client. The Coach will be available in between coaching sessions during offices hours of Monday – Friday 9am – 5pm PST by email (steph@blossomwholewellness.com ) or by telephone (505.603.6122).
B. If the Coach’s services are needed outside of the regular office hours or coaching outside of the coaching sessions, the client agrees to the $50 per 30-minute session charge.
V. Scheduling and Fees
A. This coaching agreement is in effect as of the date this agreement is signed. If paying for a monthly rate, the fee is $260 with payment collected on the first of each month. If paying for 3 months, the fee is $760, with the monthly rate after the contracted 3 months, set at $250 (with 45-minute weekly meetings). If paying for 6 months, the fee is $1400, with the monthly rate after the initial 6-month contract set at $245. All payments for the 3 or 6-month contracts will be collected at the time of contract signing. The monthly rates (after the initial contract agreement) will be collected on the first of each month.
VI. Procedure
A. The time of the coaching session and/or location will be determined by the Coach and Client based on a mutually agreed upon time. You, the Client, will initiate the call.
B. Contracted sessions (such as those within paid package contract-coaching) will be 45 minutes long. You, the Client, have the option of a video conference, phone call or in-person, where available. The exact mechanism of communication will be determined by you beforehand upon the Coach’s receipt of the “For This Session” form, which you will email to the Coach 24 hours in advance.
VII. Cancellation Policy
A. You, the Client, agree that it is your responsibility to notify the Coach 24 hours in advance if you need to reschedule.
B. If the client misses the designated time of our session without notice, you get one freebie. If you miss another session without notice, you will be charged a cancellation fee of $50. Coaching sessions will not continue until the cancellation fee is paid in full.
VIII. Termination
Either the Client or the Coach may terminate this Agreement at any time with 2 weeks (14 days) written notice. You, the Client, agree to compensate the Coach for all coaching services rendered through and including the effective date of the termination of the coaching relationship.
IX. Limited Liability
Except as expressly provided in this Agreement, the Coach makes no guarantees, representations of any kind or nature, express or implied with respect to the coaching services negotiated, agreed upon and rendered. In no event shall the Coach be liable to the Client for any indirect, consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s entire liability under this Agreement, and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to the Coach under this Agreement for all coaching services rendered through and including the termination date.
