As the parent or legal guardian of the child named below, I hereby give my full consent and approval for my child to participate in wrestling practices and instruction at High Pace Wrestling Club.

I understand that there are certain risks that may range from minor bruises to life threatening injuries or death that are inherent in the practice and instruction of wrestling, as well as in other related activities incidental to my child’s participation. I have made my child aware of these risks and by allowing him/her to participate in wrestling practice and instruction at High Pace Wrestling Club, I hereby affirm that I fully assume responsibility for these risks. I also confirm that my child is fully capable of participating in wrestling practice and instruction at High Pace Wrestling Club and that my child is healthy and has no physical or mental disabilities or conditions that would restrict full participation in these practices, except as listed below.

In addition to giving my full consent for participation, I do hereby waive, release and hold harmless High Pace Wrestling Club, its coaches, employees, and representatives for any injury that may be suffered by me in the normal course of participation in wrestling and the activities incidental thereto, whether the result of negligence or any other cause.
