If gathering data, I consent to having my hours counted and data available for use in matching grants. I understand that all data gathered as part of an AMRW program is public information, and I consent to having it publicly shared.

I assume legal responsibility for all risks involved in herring counting, including but not limited to bad weather, allergic reactions, insects, collisions, injury, or even death. I understand that it is my (or my parents, if under 18) sole responsibility to determine my fitness to participate in this program, dress appropriately, and bring what I will need to be safe and comfortable.

YOU WILL BE WALKING ACROSS A MAIN ROAD, MEASURING WATER TEMPERATURE, AND STANDING NEAR THE WATER WITH NO RAILING.

LIABILITY:

I understand the ALLIANCE FOR THE MYSTIC RIVER WATERSHED, INC. 501(c)(3) (AMRW) has invited me to participate in this program. AMRW (nor anyone volunteering or employed by AMRW) does not assume any responsibility for any loss, damage, or injury, including death to your person or property, even if it appears to have been caused by our negligence. For your own security, it is recommended that you have your own insurance or determine whether your existing insurance will cover you and your belongings. 

YOU ARE LIABLE FOR YOURSELF AND YOUR CHILDREN. IF YOU HARM SOMEONE ELSE OR DAMAGE THEIR PROPERTY, INTENTIONALLY OR NOT, YOU MAY BE HELD LIABLE BY THEM.

WAIVER:

I EXPRESSLY WAIVE (if I am a minor, the undersigned parent or guardian expressly waives) and forever release(s) to the Alliance for the Mystic River Watershed any right or rights that I have or which may accrue to me, my heirs, executors, administrators, successors, and legal representatives for compensation or damages arising out of my participation in any AMRW activities. Similarly, I 

EXPRESSLY WAIVE (if a minor, the undersigned parent or guardian expressly waives) and forever release(s) to the Town of Stonington any right or rights that I have or which may accrue to me, my heirs, executors, administrators, successors and legal representatives, for compensation or damages arising out of my participation in the River Herring Count, 2024.

HEALTH CARE CONSENT:

I hereby consent (if a minor, the undersigned parents or guardians consent) to emergency medical treatment or procedures if I cannot and agree to remain solely responsible for all related costs and expenses.

HOLD HARMLESS AGREEMENT:

I agree (if a minor, the undersigned parents or guardians agree) to AGREE TO DEFEND, INDEMNIFY, AND HOLD HARMLESS the AMRW, Town of Stonington, and anyone volunteering or employed by AMRW or the Town of Stonington from any and all claims, suits, or demands, including claims of negligence by anyone arising from my participation in the River Herring Count 2024.

PARKING LOT DIRECTIONS:

Everyone MUST park in the lot behind the Old Mystic United Methodist Church for safety and out of respect for the businesses in the area. For safety reasons, the parking lot entrance is one-way IN on the south side of the church. When exiting, head OUT by following the arrows to the north toward the playground, which will bring you back to exit onto Rt 27,  on the north side of the church. PLEASE DO NOT GO OUT THE ENTRANCE LANE. IT IS UNSAFE.

PLEDGE: I

 agree to comply fully with all directions given by AMRW program directors and group or working group leaders.
