Liability Waiver

Wild Rec LLC Waiver and Liability Release Form

In consideration for being allowed to participate in any activities, events, or programs organized or sponsored by Wild Rec LLC, I, the undersigned parent/guardian, acknowledge, appreciate, and agree to the following:
  1. Assumption of Risk: I understand and acknowledge that participation in Wild Rec LLC activities involves inherent risks, including but not limited to, the risk of physical injury, illness, or harm. I understand that these risks cannot be eliminated entirely, and I willingly accept them on behalf of my child.
  2. Release and Waiver of Liability: I hereby release, waive, discharge, and covenant not to sue Wild Rec LLC, its directors, officers, employees, volunteers, coaches, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or harm, including death, that may be sustained by my child while participating in Wild Rec LLC activities.
  3. Indemnification: I agree to indemnify and hold harmless Wild Rec LLC, its directors, officers, employees, volunteers, coaches, and agents from any and all claims, liabilities, damages, costs, or expenses, including legal fees, arising out of or related to my child's participation in Wild Rec LLC activities.
  4. Not Responsible for Events Execution or Content: I understand and acknowledge that Wild Rec LLC is responsible for helping organize and coordinate events, but is not liable for the execution or content of the events themselves, including but not limited to, games, practices, or competitions. The actual conduct and content of these events are the responsibility of the participating teams, parents, coaches, or venues.
  5. Medical Treatment Authorization: In the event of an emergency or injury, I authorize Wild Rec LLC and its representatives to obtain or provide medical treatment for my child. I agree to assume all responsibility for any medical expenses incurred as a result of such treatment.
  6. Acknowledgment of Understanding: I have read this waiver and release form, fully understand its terms, and voluntarily agree to be bound by them. I acknowledge that I am signing this document freely and voluntarily, and that by doing so, I waive certain legal rights, including the right to sue
