Please read our adoption process, BEFORE completing your application, to review our requirements. Incomplete applications will not be processed. In addition, our initial contact with you will be via email. PLEASE check your spam periodically over the first few days after submitting your application, to be sure you do not miss our initial email. Due to the volume of applications we receive, applicants that do not respond to the email may not get processed. **We recommend making your vet and personal references aware that you plan on applying to adopt a dog in the near future and that they have been listed as a reference. By submitting this application, I certify that the information I have given is true and that any misrepresentation of facts may result in my losing the privilege of adoptiong a companinon animal from For the Love of Labs Rescue. This dog will reside in my home as a pet. I will provide it with adequate food, water, shelter, training, affection and medical care. If the dog is not spayed/neutered at the time of adoption, due to age, I agree to spay or neuter the dog as soon as medically necessary, at my expense, and forward evidence of such to For the Love of Labs Rescue by the date requested. For the Love of Labs Rescue is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog in my household. Any illnesses or injuries For the Love of Labs Rescue is not aware of before adoption or that arise after adoption are solely my responsibility. I understand that there is a non-refundable, non-negotiable $495 adoption donation to help offset pulling fees, vet fees and transport. All donations are tax deductible.

I am in full agreement with these terms of adoption.
