Acknowledgment and Assumption of Risk:

I, the undersigned participant, acknowledge that my participation in activities and training sessions with C2S Volleyball LLC conducted at University Lake School involves inherent risks, including but not limited to physical injury, property damage, or other harm. I voluntarily assume all risks associated with my participation in these activities.

Release and Waiver:

In consideration of being allowed to participate in activities and training sessions with C2S Volleyball LLC conducted at University Lake School, I hereby release, waive, discharge, and covenant not to sue C2S Volleyball LLC, its owners, directors, officers, employees, agents, and representatives (collectively, the "Released Parties"), as well as University Lake School, its owners, directors, officers, employees, agents, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in activities and training sessions with C2S Volleyball LLC conducted at University Lake School.

Indemnification:

I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, actions, or causes of action, including attorney's fees, arising out of or related to my participation in activities and training sessions with C2S Volleyball LLC conducted at University Lake School.

Medical Treatment:

I consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in activities and training sessions with C2S Volleyball LLC conducted at University Lake School. I understand that I am responsible for all costs related to such medical treatment.

Governing Law:

This waiver and release shall be governed by and construed in accordance with the laws of the state of Wisconsin.

Acknowledgment of Understanding:

I have read this waiver and release of liability and fully understand its terms. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
