I hereby give my consent as the Parent/Guardian of the mentioned child participate in The Emotion, Body, and Belief Code session(s), as taught by Dr. Bradley Nelson. I understand that the services provided by my Practitioner, Janice Sestrap, are intended to identify and eliminate trapped emotions, underlying imbalances and belief systems by releasing trapped emotions; clearing energetic imbalances in the areas of energy, circuitry, pathogens, structure, toxicity, and nutrition; and identifying and releasing unwanted beliefs. These methods of energy healing promote harmony and balance within, relieving stress and supporting the body’s natural ability to heal. Energy healing such as these methods is widely recognized as a valuable and effective complement to conventional medical care.

I understand that releasing trapped emotions, belief systems, or the correlation of any other energetic imbalance using these methods as practiced by my Practitioner, Janice Sestrap, is not a substitute for traditional medical treatment or psycho therapeutic. This information is not intended as medical advice and should not be used for medical diagnosis or treatment. I am fully aware that my Practitioner, Janice Sestrap, will not offer any diagnosis or recommend any medical treatment or prescribed medication for my child.

I understand that I must continue to ensure my child has regular medical check-ups as part of their overall personal health care plan; and I should contact my own certified and licensed medical physician/doctor/health care professional for any physical or psychological ailments or concerns that I may have to get proper medical advice for my child.

I agree and understand that my child’s participation in The Emotion Code and/or The Body Code session(s) is voluntary and that at any time during a session I can choose to end my child’s participation. I also understand that in approximately 20% of sessions, the release of trapped emotion(s) or other energy(s) may result in “processing” where echoes of the emotion(s) or other energy(s) released may manifest in temporary physical or emotional discomfort, and that this “processing” appears to be a normal part of regaining energetic balance.

I understand that any information exchanged during any session is educational in nature and is to be used at my own discretion. My Practitioner, Janice Sestrap, makes no claims as to healing or recovery from any illness my child may have now, nor the prevention of any illness my animal may have in the future, and that no guarantee is made towards validity. I also understand that any information imparted during these sessions is strictly confidential and will not be shared with anyone without my written permission.

Finally, I understand that by providing this informed consent I am assuming full responsibility for my child participating in The Emotion, Body, and Belief Code session(s) and I hold harmless my Practitioner, Janice Sestrap. I agree to the terms and conditions set out within The Emotion, Body, and Belief Code Consent Form and certify that the above information is true and correct.
