Dysautonomia Louisiana is making waves and holding their 3rd Annual 'Fishing for a Cause' Angler Rodeo at Campo's Marina in Shell Beach, La May 23rd & 24th, 2025 Dysautonomia Louisiana, Inc. is a non-profit organization. All funds raised will help our organization spread awareness about dysautonomia across Louisiana. We appreciate your support and look forward to having some fun, fishing for a cause!

2025 Dysautonomia Louisiana 'Fishing for a Cause'

WAIVER, RELEASE OF LIABILITY, and INDEMNITY AGREEMENT

I HEREBY WAIVE, RELEASE, AND DISCHARGE from any and all liability, Dysautonomia Louisiana, Inc., it's members, agents, assigns, and employees ("Released Parties") from any liability arising from the negligence or fault of Released Parties or any other entity or individual for any accident, injury, death, property damage, property theft, or any other claims or actions of any kind which may occur to me or my minor children, including travel to and from the activity, whether by automobile or watercraft.

I HEREBY AGREE TO DEFEND, HOLD HARMLESS, AND INDEMNIFY Released Parties from any and all liability and/or claims made by me or my heirs as a result of participation in this activity, whether caused by the negligence of me, my minor child, or any other individual or entity. This includes being responsible for any attorney fees and costs associated with such indemnity obligation assumed under this Agreement.

I acknowledge that this activity involves the potential and risk of injury, which includes risks associated with being on a watercraft, temperatures, weather, which are inherent to all participants.

I understand that while participating in this activity, me and/or my minor child may be photographed or video taped. I agree to allow any such photographs or video to be used for any legitimate purpose by Released Parties.

This WAIVER, RELEASE, AND INDEMNITY AGREEMENT is to be construed broadly to provide waiver, releasae, and indemnity to the maximum extent permitted under the Laws of Louisiana. The Agreement is to be construed under the Laws of Louisiana, and any action challenging any aspect of this Agreement shall be brought in the 22nd Judicial District Court for the Parish of St. Tammany, State of Louisiana, only.

I CERTIFY THAT I HAVE READ THIS AGREEMENT, I FULLY UNDERSTAND THE CONTENT OF THE AGREEMENT, AND I HAVE SIGNED OF MY OWN FREE WILL.

**The submition of this registration form represents acknowledgement that I agree to sponsor the 2025 Dysautonomia Louisiana Fishing Rodeo. I acknowlege that this includes the following: my name/logo on the entry banner, my logo on the Dysautonomia Louisiana website and social media pages. I agree to payment before May 1st, 2025.
