Boarding Consent Form

Please complete the form below and submit it for review. Registration forms, consent forms, & current vaccination records are required to be submitted 15 days prior to your scheduled date(s) of service. Good Pup Club reserves the right to cancel and refund any scheduled service(s) if the required registration forms, consent forms & current vaccination records have not been received 15 days prior to the scheduled date(s) of service. Additionally, all dogs must have a complete physical with a negative fecal within 72hrs prior to the initial boarding day. No exceptions will be made. Good Pup Club reserves the right to cancel and refund any scheduled service(s) if a complete physical with a negative fecal has not been completed 72hrs prior to the initial boarding day.

OWNER'S RESPONSIBILITY The Owner, not Good Pup Club, nor it's employees, will be responsible for providing the dog(s) food for the duration of the boarding. The Owner will also be responsible for providing any special treats that may be necessary for allergy or religious accommodations. 

FINANCIAL All boarding fees are to paid in full 15 days prior to the initial date of service. Any late cancellations with less than 72hrs notice will be subject to a 25% cancellation fee, plus any holiday fees, charged at Good Pup Club's discretion. Failure to pay boarding fees 15 days prior to the initial date of service will result in an automatic cancellation of services.

HOLIDAY FEE On holidays, and the days surrounding them, an additional $75/day fee will be charged prior to boarding. Holiday fees are non refundable if the Owner late cancels with less than a 72hrs notice.

EMERGENCIES In case of emergency, Good Pup Club will take your dog(s) to either to a local veterinarian's office, or a local emergency animal hospital agreed upon by the Owner and Good Pup Club prior to boarding. The Owner, not Good Pup Club, nor it's employees, will be responsible for all bills that may occur. The Owner will be responsible for leaving a form of payment either with Good Pup Club, or on file at the designated local veterinarian and local emergency animal hospital.

Good Pup Club will make attempt to notify the Owner or designated emergency contact, via phone, text, and/or email in case of an emergency. If the Owner, or designated emergency contact, is out of reach/contact Good Pup Club may have to make a decision regarding your dog(s) healthcare. By signing below you are granting Good Pup Club, and it's employees, the independent right to make medically necessary decisions for your dog(s) in case of an emergency. These decisions will be based on the recommendations from the veterinarian at the designated veterinarian's office and/or emergency animal hospital. 

LIABILITY All behavioral advice and training suggestions are provided at the Owner's own risk. Use of this information is voluntary and neither Good Pup Club, its employees, nor Trainer is responsible for injury to another human or animal. Owner expressly understands, acknowledges and agrees that Owner is at all times responsible for the actions of the Dog(s). Further, Owner, and not Good Pup Club, its employees, nor Trainer is responsible for any injuries or expenses resulting from socialization or training of the Dog(s). Owner further understands and acknowledges that dogs participating in physical exercises may be injured by running, jumping, chasing a ball etc. Accordingly, Owner expressly hereby promises and warrants that owner shall indemnify and hold harmless the Good Pup Club, its employees, and Trainer for any and all such injuries.
