Membership Hold Request
Please complete this form to request that your membership be put on hold. Hold periods can be for between 1-6 months There is a $10 hold fee If you have a payment scheduled just before your hold date, that payment will be deducted in full, but your next payment date will be delayed by the period that your membership is on hold. Your class credits for that payment cycle will be available for use when you come off hold. If you wish to attend a class during the hold period, you can purchase a casual class It is your responsibility to remember when your hold ends. If you need to extend it please email us at info@dragonflydance.com.au at least one week before your hold is due to end.

I understand and agree to the following dot points
I understand that:
two weeks notice is required to ensure the hold is put in place in time
if a payment is due before my hold period, the class credits will be held on my account until the hold is finished (payment amounts will not be adjusted)
I am responsible for cancelling any pre-booked classes during the hold period
I am responsible for advising my teacher that I will be away
I am responsible for knowing when my hold is due to end and that if I want to extend it I need to fill our the '
hold extension request' form
The hold is not in place until I have received a confirmation email
Holds cannot be applied for in retrospect except in extenuating circumstances
I will be charged a $10 fee for putting my membership on hold (as per
