Agreement to Treat

This agreement is valid upon signing and completion of all checkboxes. The agreement to treat form is valid for all pets within the household and only needs to be completed by the Client once.

I, the Client, herby agree to allow Pets and Wildlife Services AU (PAWWSAU) to treat, care and perform services upon my request for my pet(s)

I, the Client, am aware that Pets and Wildlife Services AU (PAWSAU) may cease the treatment, care or service at any time should my pet become anxious, stressed, aggressive or show potential injury to themselves or others

I, the Client agree that should the treatment, care or service cease due to the above reasons that 50% of the total service fee is still payable to Pets and Wildlife Services (PAWSAU)

I, the Client, agree that Pets and Wildlife Services AU may use the necessary stress free restraints at the discretion of the staff member performing the service should it be required

I, the Client agree that in the event of injury to Pets and Wildlife Services AU (PAWSAU) staff that my details may be shared with the Companies insurance and healthcare officials

I, the Client, understand that a deposit of 50% is required at the time of booking and the remaining 50% payable at the time of service
