In consideration of being allowed to enter and participate in any "Activities" at The Playful Family, Inc. ("Premises"), the undersigned acknowledges, understands, and agrees to the following:

1.  I acknowledge that all applicable rules for participation must be followed, and I assume sole responsibility my personal safety.

2.  I understand that The Playful Family, Inc. management may refuse my volunteer participation in Activities for any reason, including, but not limited to:

a) Failure by me to follow The Playful Family, Inc rules.

b) My failure to remain on the Premises during volunteer participation in the Activities.

c) My failure to adequately participate safely in volunteer Activities.

d) The Child suffering from an illness or ailment of any kind that may impair his or her continued participation in the Activities, or that may be contagious or potentially harmful to others on the Premises.

3.  I, at this moment grant The Playful Family, Inc. and contracted photographers hired by The Playful Family permission to take pictures of myself and/or video, including but not limited to:  media, marketing, publicity, copyright purposes, illustration, advertising, social media, and web content.

4.  As a volunteer, I will participate in various activities offered by The Playful Family, Inc., including but not limited to the following:  The Playful Playground indoor play center that is fully equipped and/or the participation in any party, event or program activity at The Playful Family, Inc.

5.  As a volunteer Participation in the Activities carries with it certain inherent risks which cannot be eliminated regardless of the care taken to avoid injuries. The specific risks may range from contagious illnesses, including COVID-19, minor scratches, bruises, and sprains, to more serious injury, including disability and/ or death.  Notwithstanding these risks and other hazards that may be foreseeable but not specifically identified here, I, for myself and respective heirs, personal representatives, and assigns, understand, acknowledge, and expressly and voluntarily assume all risks and full responsibility for any injury to Child or myself, and expressly and arising out of related to the Activities.  I hereby irrevocably waive, release and fully discharge The Playful Family, Inc. from any and all liability for loss, damage, personal injury, including death and disability, and costs of any kind and nature whatsoever which I may suffer now or in the future by reason of or existing out of any causes whatsoever related to or arising out of or from the participation of the Volunteer activities associated with The Playful Family, Inc. on and off the Premises.

6.  Indemnification and Hold Harmless:  I agree to indemnify and hold The Playful Family, Inc. harmless from, without limitation, any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees and costs incurred due to claims brought by any third party as a result of or arising out of my own involvement in the Activities and to reimburse The Playful Family, Inc. for any such costs, expenses, and fees as they are incurred.

7.  Release, Discharge, and agree not to sue:  I, for myself and respective heirs, personal representatives, and assigns, do hereby release, discharge, and agree not to sue The Playful Family, Inc. and its managers, members, employees, and/or other agents, for any injury to or death of myself arising, directly or indirectly, from participation in the Activities.  This release, discharge, and covenant not to sue shall relate to any and all claims or legal rights now existing or arising in the future, including claims and legal rights arising out of any negligence of The Playful Family, Inc. and/or its managers, members, employees, and/or other agents and any other breach of a legal duty arising out of common law, statute, contract or otherwise.

8.  Severability:  This document is intended to be as broad and inclusive as is permitted by the laws of the State of Florida, and if any provision (or a part of any provision) contained herein is deemed to be invalid, the balance of the conditions shall continue in full legal force and effect, notwithstanding such invalidity.

I HAVE READ THIS AGREEMENT THOROUGHLY.  I UNDERSTAND THAT, BY SIGNING THIS AGREEMENT, I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I, AND OUR RESPECTIVE HEIRS, ASSIGNS, EXECUTORS, ADMINISTRATORS, PERSONAL REPRESENTATIVES, AND NEXT OF KIN MAY HAVE AGAINST THE RELEASEES, INCLUDING THE RIGHT TO SUE.  I SIGN THIS DOCUMENT VOLUNTARILY AND WITHOUT INDUCEMENT.
