Withdrawal Request Form Fields marked with an * are required INSTRUCTIONS: Withdrawal is a permanent removal from Excite! We offer temporary freeze options if you are looking for a short term break due to vacations, holidays, other conflicts. Please see our Account Freeze Request form for details. This form is for all classes and includes: Moms Day Out and Lunch Bunch. Team members are under contract and are not eligible for a 30 day paid withdrawal. 30 Day paid notice is required from each Excite! student. If you have more than one student enrolled please submit a separate request for each student.
