OKLAHOMA CITY BEAUTIFUL, INC.  organizes volunteers to implement public and private projects to improve the beauty and sustainabilty of our community. As a condition of participation in an OKLAHOMA CITY BEAUTIFUL, INC. program, I execute this Release and Waiver.

I FULLY UNDERSTAND that there is a risk whenever I am in a park area, median, street right-of-way, or near traffic and streets. I FURTHER FULLY UNDERSTAND the risks of participating in projects involving sharp tools, physical activity, and in the proximity to vehicular traffic; hidden and latent objects, insects and holes.

I AGREE to stay off City streets while implementing projects; to be alert to traffic on, entering and exiting my designated site or on City streets; to avoid grass cutting and construction activities on or nearby these locations; and to follow the instructions, terms, conditions and recommendations from OKC Beautiful and the City of Oklahoma City.

I UNDERSTAND with regard to my volunteer activities under the OKC Beautiful programs that I am not employed or an agent of OKC Beautiful or the City of Oklahoma City and I am not covered by any insurance or workmen’s compensation coverage by my participation in a volunteer program.

I DO HEREBY RELEASE and forever discharge and hold harmless Oklahoma City Beautiful, Inc. (“OKC Beautiful”) and the City of Oklahoma City and their officers, agents, employees, successors and assigns from any and all liability claims and causes of action for any damages and/or injuries which may result from my participation in the OKC Beautiful and City of Oklahoma City volunteer programs.

I AGREE TO HOLD HARMLESS OKC Beautiful and the City of Oklahoma City and its officers, agents and employees from liability for any damages or injuries resulting from any acts or failure to act on my part during my participation in the OKC Beautiful and the City of Oklahoma City volunteer programs. I also understand that OKC Beautiful does not assume any responsibility or obligation to provide financial or other assistance, including, but not limited to, medical, health, or disability insurance, in the event of injury, illness, death, or property damage.

Photography/Audio release. I do hereby grant and convey unto OKC Beautiful all rights, titles, and interest in and to any and all photographic images and video or audio recordings made by or on behalf of OKC Beautiful, or made with its consent, during my participation in a volunteer project and/or any project, activity or event sponsored, managed, arranged, or promoted by, or otherwise affiliated or associated with OKC Beautiful, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.

BY COMPLETING THIS FORM, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THIS RELEASE AND WAIVER AND AGREE TO ABIDE BY ITS PROVISIONS.
