GENERAL MINORS RELEASE and WAIVER with CONTACT INFORMATION

This form ust be completed and turned in before the start of any classes, workshops and programs your child is registered to attend with Paint'n With Friends (aka PWF). Please fill out a separate form for each child. This Release/Waiver will be on file throughout the duration of the current year, and must be completed each calendar year your child attends events with Paint'n With Friends. Thank you.

ABILITY TO ENGAGE IN ACTIVITY AND ASSUMPTION OF RISK:

PWF takes all precautions to reduce risk and provide a safe, healthy and enjoyabl experiences. I (parent or guardian) warrant that my child is able to follow directions for all activities while engaged at this event with PWF. I acknowledge that risk from participation in class activity exists and that I have allowed my child to attend this event knowing these risks and possible consequences including, but not limited to personal injury.

WAIVER & RELEASE OF LIABILITY:

As parent or guardian of my child, listed below, I agree not to hold Paint'n With Friends not liable for any personal injury, property damage or loss of insurance. I agree to release and hold harmless Paint'n With Friend and Tomasa M. Brown from all liability incurred as a result of my child's participantion in any classes, workshops, programs etc and that these terms serve as a release for Tomasa M. Brown, volunteers, property owners and members of my family.
