1.  Please take care of our equipment as if it were your own.

2.  Please make sure pets are current on vaccinations and do not have signs of illness or infestations of fleas or ticks.

3.  No aggressive dogs!!

4.  Do not leave your pet unattended at any time.  Dogs must be secured at all times while in the tub.

5.  Please use grooming products sparingly.

6.  Children must be with an adult when using the facility.

7.  Wash times are not regulated.  Please be aware of others waiting to use the facility and limit your wash time if necessary.

8.  Please clean and disinfect the tub after each use.  Leave it clean for the next pet to enjoy.  Place all used towels in the provided hamper.

9.  Do not share your subscriptions with friends and family.  Use the facility for only those dogs listed.

10.  Please remember that there may be other pets in the building, therefore for the safety of all, keep your pet in your control at all times.

11.  Please help us keep our facility clean and friendly so that everyone enjoys their time here.

I understand that due to the nature of the business Muddy River Dog Washes' floors may be slippery when wet and I will exercise caution within the facility.

I will closely supervise my pet at all times during our visit to Muddy River Dog Wash.  I understand that any items damaged by myself or my pet, are my responsibility and I will be held fully responsible for their cost.

I agree to keep my dog leashed at all times during our visit to Muddy River Dog Wash.  I also agree to keep my dog secured with a tie at all times while in the tub.

I certify that my pet is current on all vaccinations including Rabies, Distemper and Parvovirus.  Bordetella vaccination is highly recommended.  I also agree that if my pet is showing any signs of illness, I will not use the facilities until my pet is healthy again.  I agree that if my pet is aggressive, I will not use the facilities.  I agree that Muddy River Dog Wash has the right to refuse service if any of the above mentioned is found to be violated.  I also agree that if a subscription was purchased and service is refused, that I will not receive reimbursement for any of the above-mentioned reasons.

I understand that if a subscription is purchased that I will only use it for the pets listed and I will not share it with my friends and family.  If I violate this, my subscription will be terminated and I will not be reimbursed for the remainder of the subscription.

I understand that Muddy River Dog Wash is open 24 hours a day and is not staffed.  The property is monitored with 24 hour security cameras.  The use of these facilities is at your own risk.

I understand the risks of using Muddy River Dog Wash with my pet.  Such risks may include property damage or physical injury inside or outside the facility, such as falling, slipping, illness or dog bites.  I voluntarily release and agree to hold harmless Muddy River Dog Wash, Holbrook Veterinary Clinic and Reidhead Rentals and its agents from any liability, claims, demands which are related to or in any way connected with my pets using the wash facilities and Muddy River Dog Wash.
