I the undersigned allow Lumina  Laser Lab Tattoo Removal the use of any video/images of the process or progress of my treatments for the use on social media, website, advertising andto monitor progress thereof.

I acknowledge that I have chosen to undergo laser tattoo removal treatment with ful understanding and acceptance that burns are a possible side effect of the treatment.

I understand that laser tattoo removal involves the use of a high-energy laser that may generate heat, which can cause burns or other damage to the skin. 

While every effort will be made to minimize the risk of burns, I acknowledge that this risk exists and that I accept this risk.

I understand that the severity of the burn may vary depending on various factors, such as the colour, age and location of the tattoo, as well as my skin type and medical history. 

I also acknowledge that burns may result in scarring, permanent skin discoloration, or other complications.

I agree to follow all aftercare instructions provided by my laser technician to minimize the risk of burns or other side effects. 

I understand that failure to follow these instructions may increase the risk of burns and other complications.

I release Lumina Laser Lab and their staff from any and all liability for any injury or damage that may occur as a result of the laser tattoo removal treatment, including burns or other side effects.
