CLIENT RISK ACKNOWLEDGMENT & SAFETY GUIDANCE FORM

This form must be completed by all climbers before participating in any Treetop Rocks CIC session or activity. If the climber(s) (is/are) under 18 years of age, this form must be completed by a PARENT, or LEGAL GUARDIAN, or by A PERSON WHO HAS BEEN GIVEN PERMISSION TO SIGN BY THE PARENT/LEGAL GUARDIAN OF THE CLIMBER. This means community group leaders may sign for children on behalf of parents. RISK ACKNOWLEDGMENT PARAMETERS

  • There will always be risks involved in any outdoor activity and the benefits of the activity are often contingent on  the participant's experience of supported risk management by the event operator.
  • Treetop Rocks CIC offers recreational tree climbing (RTC) in a controlled risk environment, with a robust risk management protocol in place to mitigate the risks of injuries, lost or damaged property, and other adverse events. 
  • While Treetop Rocks CIC makes every effort to minimise risks, we will not be held liable for accidents, death, injury that are not a result of negligence on the part of Treetop Rocks CIC, or for any accidents or losses which are due to participant’s failure to follow the instructions of the instructors.
  • No person should be under the influence of alcohol before or during the activity. If a person is found to be under the influence of alcohol (including from the night before)  it will be the instructor’s discretion whether this person will be able to take part in the activity. No monies will be refunded if the person does not take part.
  • Treetop Rocks CIC does not guarantee the security of any property including vehicles brought to our sites and no responsibility will be taken for loss or damage. Property belonging to the customer is at all times the responsibility of the customer.

SAFETY INFORMATION

Please discuss this safety information with all climbers in your care:
  • Please bring a WATER BOTTLE - climbing is thirsty work.
  • LONG HAIR must be TIED-UP.
  • JEWELLERY such as necklaces or large earrings should be REMOVED
  • If you have GLOVES & want to use them to protect from blisters, please bring them - even woollen gloves are fine. However the majority of people prefer to climb without them
  • All FOOTWEAR should have a PROTECTED TOE area and should be suitable for sport, e.g. TRAINERS.
  • Ideally WEAR LONG TROUSERS & LONG SLEEVES & ensure you have weather-appropriate clothing.
  • AVOID GROUND-based FEATURES in the area leading to & surrounding the tree, including rocks, stones, loose soil, tree stumps, tree roots.
  • AVOID DOG MESS. Because hands & feet touch the same ropes,  for your own safety we cannot let you climb with it on your shoes.
  • YOU MUST LISTEN TO THE LESSON CONTENT & FOLLOW ALL INSTRUCTIONS & GUIDANCE TO THE BEST OF YOUR ABILITY. You are learning a new skill which is  challenging & requires full-focus.

By signing this form I agree to the following:

  • I confirm that I am over 18 years of age and wish to participate in a Treetop Rocks climbing session or that I am over 18 and give my consent to those in my care to participate in a Treetop Rocks climbing session
  • I confirm that I am a parent or guardian of any children listed below or that I have the authority to sign this form on behalf of the parent or guardian
  • I confirm that any child participating in this activity is 3 years old or above, unless otherwise agreed by the instructor
  • I confirm that I am accepting the provisions of this Risk Acknowledgement either for myself or for those in my care for whom I am giving permission to participate
  • I accept, for myself or for those in my care for whom I am giving permission to participate, that there are risks attached to this activity including but not limited to: tripping over tree roots & stumps, rope burns, falling branches, equipment failure due to improper use by clients, medical occurrences during climbing
  • I declare that I, or those in my care for whom I am giving permission to participate, (am/are) participating in a Treetop Rocks session at (my/their) own risk, & with an awareness of these risks, & acknowledge that Treetop Rocks CIC accepts no responsibility for injuries or death resulting from any person’s involvement in our sessions, except where injury or death is caused by the negligence of Treetop Rocks CIC & its staff
  • I, or those in my care for whom I am giving permission to participate, accept that all personal property including vehicles is at all times the responsibility of the customer & that Treetop Rocks CIC accepts no responsibility for loss or damage to such property
  • I, or those in my care for whom I am giving permission to participate, will follow all guidance & safety instructions given by Treetop Rocks’ staff
  • I, or those in my care for whom I am giving permission to participate, accept responsibility for avoiding ground-based features in the area leading to & surrounding the tree, including rocks, stones, loose soil, tree stumps, tree roots, climbing equipment & PPE
  • I understand that I, or those in my care for whom I am giving permission to participate, must remove all items including mobile phones from pockets, remove all jewellery & accessories, & tie back long hair for (my/their) own safety
  • I guarantee that I, or those in my care for whom I am giving permission to participate, will not be under the influence of alcohol or drugs (including from the night before) on the day of the session
  • I acknowledge that this is a reasonably strenuous activity with a weight limit of 100kg & confirm that I, or those in my care for whom I am giving permission to participate, (am/are) medically fit to take part & have made the Treetop Rocks CIC instructors aware of any illnesses, disabilities, or additional support needs
  • I confirm that I, or those in my care for whom I am giving permission to participate, (am/are) not pregnant & that if (I/they) climb whilst pregnant (I/they) assume all risk to (my/their) health & that of (my/their) unborn child
  • I declare that I, or those in my care for whom I am giving permission to participate, will follow any applicable Covid 19 Risk Prevention Measures set out on the Scottish Government website: https://www.gov.scot/publications/coronavirus-covid-19-staying-safe-and-protecting-others/
  • I, or those in my care for whom I am giving permission to participate, understand that with respect to general data protection regulations (GDPR), photos showing other people’s children may not be shared on social media or anywhere else without their permission
  • I give Treetop Rocks Permission to take photos to use for their marketing and PR. (You may disagree with this clause by writing “No photos”  next to the climber's name)
