I, the undersigned, declare that I am the legal parent or guardian of the child(ren) identified above (the "Participant(s)") and am fully competent to sign this Agreement. By signing below, I agree to release and hold harmless Studio Anthurium, LLC (the "Studio"), its owners, operators, members, managers, employees, agents, and contractors from any and all liabilities, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant(s) and/or myself while in or upon the premises of the Studio or any premises under the control of the Studio's owners and/or operators, or while en route to or from any such premises.

Acknowledgment and Acceptance of Risks:

1. I acknowledge the existence of both known and unknown risks associated with participation in Studio activities, which include, but are not limited to, contusions, fractures, scrapes, cuts, bumps, paralysis, or death.
2. I willingly assume all such risks, including those that may arise from the actions of other participants.

Conditions of Participation:

1. The Participant(s) and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions of participation in any Studio events.
2. I understand and agree that the Studio is not liable for any injuries, liabilities, or damages from participation, except those arising from gross negligence or willful misconduct by the Studio.

Indemnification:

1. I agree to indemnify the Studio and its affiliates for any defense costs or expenses arising from any claims related to the Participant's involvement in Studio activities, except those arising from the gross negligence or willful misconduct of the Studio.

General Provisions:

1. Consent for Use of Likeness: I consent to the Studio's use of any photographs, film, video, or likeness of the Participant(s) for any legal purpose.
2. Arbitration: Any disputes under this Agreement will be resolved by binding arbitration in San Mateo County, California, according to the rules of the American Arbitration Association.
3. Severability: If any provision of this Agreement is found invalid or unenforceable, the remaining provisions will continue to be valid and enforceable.

Affirmation of Agreement: I have read this Agreement thoroughly and understand that by signing it, I give up substantial rights in exchange for the participation of the Participant(s) in Studio activities. I sign this document voluntarily and without any inducement.
