Liabilty Waiver

I hereby acknowledge that my participation/my child's participation in activities organized by LACE SOCCER LLC involves inherent risks associated with soccer and sports activities. In consideration for being permitted to participate in these activities, I agree to release and discharge LACE SOCCER LLC, its officers, employees, agents, and representatives from any and all claims, liabilities, damages, or injuries that may arise during or as a result of my participation/my child's participation, regardless of whether caused by the negligence of the aforementioned parties or otherwise.

Photo Consent

I understand and agree that LACE SOCCER LLC may use photographs, video recordings, or other media of myself/my child taken during these activities for promotional purposes, including but not limited to website content, social media, and marketing materials. I waive any right to inspect or approve the finished product wherein my/my child's likeness appears. I also understand that the images may be used without compensation or notification. I acknowledge that this waiver and consent form applies to all activities and events organized by LACE SOCCER LLC.

I certify that I am of legal age and competent to sign this waiver and consent form.
