The acceptance of this application is at the sole discretion of Altitude Music Festival Management. I acknowledge that this form serves as an application and does not guarantee a team slot until acceptance confirmation is received. By submitting below, I confirm that I am authorized to submit this Application Form on behalf of the aforementioned ("Client") and agree to the Terms and Conditions outlined in the ("Terms & Conditions").

This application does not secure place on the team until an acceptance email is received from Altitude Music Festival. If Altitude Music Festival approves this Application Form, the applicant agrees to abide by the Terms and Conditions, complete any required paperwork, and fulfill the responsibilities outlined, including ambassador representation and community engagement as part of Street Team Member duties. The team member placement is at the sole discretion of Altitude Music Festival.

Any undefined capitalized terms in this Application Form will be interpreted according to the Terms and Conditions. If the Application Form is not accepted by Altitude Music Festival, any prepaid fees will be refunded.
