I understand and voluntarily assume all risks associated with participating in physical activities at Siris Art Studio LLC, including but not limited to aerial hammock, Lyra, flexibility, aerial silk, and pole classes. I recognize that these activities involve inherent risks of physical injury, and I agree to assume full responsibility for my safety and any resulting injuries.

I release and discharge Siris Art Studio LLC, its owners, instructors, employees, and agents from any claims or liability for injury, loss, or damages resulting from participation in studio activities. This release includes claims of ordinary negligence.

 I agree to indemnify and hold harmless Siris Art Studio LLC from any claims, damages, or expenses (including legal fees) resulting from my participation or my minor’s participation in activities at the studio. I understand that this indemnification extends to claims made by third parties as a result of my actions or those of my minor.

In the event of an emergency, I authorize Siris Art Studio LLC to seek appropriate medical treatment on my behalf or that of my minor, understanding that I am responsible for any associated costs.

1. Eligibility I confirm that I am at least 18 years of age and fully eligible to participate in classes at Siris Art Studio.
2. Health and Safety  I confirm that I am in good physical health and have disclosed any pre-existing medical conditions or injuries that may affect my participation in pole dance or aerial yoga classes. I will immediately inform the instructor of any changes to my health status.
3. Liability Waiver  I agree to release Siris Art Studio, its staff, and instructors from any liability for injuries or accidents that may occur during classes, including those related to falls, muscle strain, or other risks inherent to pole dancing and aerial yoga. I acknowledge that participation is voluntary and assume full responsibility for any injuries sustained.
4. Class Scheduling and Cancellations  I agree to notify the studio at least 24 hours in advance if I am unable to attend a class. Classes missed without notice will not be refunded or rescheduled. Cancellations with less than 24 hours' notice may result in the forfeiture of class fees.
5. Payment and Refunds  I agree to pay for classes in full at the time of booking. No refunds will be issued for unused classes, missed sessions, or early termination of services. If a class is canceled by the studio, a credit or refund will be provided.
6. Dress Code I agree to wear appropriate attire for pole dance classes, which includes clothing that allows for freedom of movement and adequate grip on the pole. Shoes, jewelry, or clothing that could interfere with the class or safety are not permitted. Hair must be tied back securely.
7. Studio Conduct I agree to behave respectfully and professionally at all times in the studio. I will refrain from inappropriate language, behavior, or any actions that could disrupt the class or cause harm to myself or others. I understand that failure to comply with studio policies may result in being asked to leave without refund.
8. Use of Studio Equipment  I understand that all equipment provided by the studio must be used according to the instructor’s instructions. I will not attempt to use equipment without guidance, and I accept full responsibility for any misuse of the studio’s facilities.
9. Photography and Media Consent I consent to Siris Art Studio taking photographs or videos during class for promotional purposes, which may include posting on social media or the studio website. I understand that my image may be used without compensation.
10. Drop Procedure  I agree to notify Siris Art Studio in writing via email or in person if I wish to discontinue classes. If I do not formally notify the studio, my account will continue to be charged until the studio receives notice of my cancellation.
